Additional information

  • Replacement documents cost is $21.68*
  • You will receive a request for payment once we have located your historical record.
  • Upon receipt of your payment, your request will be processed within 10 working days.
  • If you are experiencing financial hardship or special circumstances, please let us know when you submit the form.

The types of historical records that can be issued by the Department of Employment, Small Business and Training (DESBT) are listed in this table:

Qualification certificate

A qualification certificate issued by a registered training organisation (e.g. Certificate III in Carpentry)

Refer to your registered training organisation (RTO).

Visit Training.gov.au to search providers registered to deliver nationally recognised training.

Note: If your RTO closed before 30 June 2012, complete the Historical records request form (PDF). DESBT may hold a copy of your qualification certificate.

Statement of results

A statement of results is an equivalent transcript issued to school age students (not studying year 12 in school) or adult learners eligible for a Queensland Certificate of Education (QCE)
